EEA PR Online Application

I would be very grateful for some help on the following case:
I am Bulgarian national, came to UK in January 2007, as self-employed and two years later became a student. I had yellow card from HO confirming self-employment status and with the same card I could study without the need to obtain permission from HO.
In January 2012 I have applied and received the Blue Registration Certificate that gave me free access to the labor market and since then I am working as employee until now, so total 6 years.
Now I am applying for EEA PR and I am really confused with the online application. I am applying on a basis of having been working for 6 years lawfully, but was wondering do I have to mention my self employment and student status, even if this is out of the qualifying period? The questions are:
"Have you been self-employed since entering the UK?"
"Have you been a student or done vocational training since entering the UK?"
At the end of the application there is a list of documents I have to provide and turns out, that as if I mention self-employment and student status they require evidence for those too... So do I answer yes or no to the above questions, as you can see where the confusion is coming from.. they did not mention for the qualifying period, but since I have entered UK
Also the following question:
"Have any of your EEA residence documents been revoked or cancelled by the Home Office?" .. My yellow card has been taken from HO, claiming I have studying and working without authorization from HO, in which case was HO's mistake. I have appealed against the decision and HO admitted their mistake, following the issue of Blue Registration Certificate. Do I answer yes or not to this?
Authorization documents - do I mention only the Blue card or the Yellow one too? again this is out of the qualifying period if I have to do that..
"Did you work for an uninterrupted period of 12 months in accordance with one or more of the documents mentioned in the previous question?"
"When did you complete this 12-month period?"
