Family Permit After Expired Residence Card

Hello
I am a German citizen and my wife is a Pakistani passport holder. She had a residence card by virtue of being a family member of an EEA national for 5yrs that expired in October. She travelled to Pakistan without us realizing that her residence card expired so has been advised to apply for an EEA family permit to re-enter the UK.

I had some confusion when completing the form that I would appreciate any insight on:
1) It asks for her residential address. She has been resident in the UK but is applying from Pakistan so what should she enter?
2) It asks if we live together which we do but if the address in (1) above is Pakistan and I am resident in the UK then how should we answer this?
3) We have a daughter who is not FINANCIALLY dependent on my wife. Should she be included under the question about any financial dependents?
4) The application asks if she can provide my passport. I can send her a certified copy so can she answer yes?
5) There is no place in the application to indicate that my wife has previously held a residence card and was also previously approved for an EEA family permit: should I include this somewhere?
