EEA RESIDENCE CARD FOR 2YEARS BABY

Hi guys,
Please i need some advise, my EU partner is applying for her registration certificate and she want to add a NON Eu dependant child staying with us in the application.
So the question is, As she is about to send her Registration certificate application to HM with the paper form, does the child need separate application form ?
Secondly, does the 2years old child need to provide two passport photos ?
