ILR: Class 2 and 4 NI, three sources of Income

Dear Helper,
Kindly advice if I can use three sources of income for ILR, stated below
Employee (Only 5 Months) April 2012 - August 2012
Self Employed (Only 5 Months) November 2012 - March 2013
Limited Company Dividend (12 Months) April 2012 - March 2013.
Secondly: Kindly advice on the income evidence if this is okay:
Employee (Bank Statement and Payslip or Employer's letter)
Self Employed (Bank Statement and Payslip))
Limited Company Dividend (Accountant's Letter and Invoice Explanation or Dividend Voucher)
Other Documents:
- Company's profit and loss account
- Bank statement
Thirdly:
Must I pay class 2 and 4 NI as both Self Employed and Director of Limited Company?
