Wife+Child of a BC/Settled Applying visa from India

Main Applicant (Myself) - British Passport holder
Dependent 1 (Wife) - Lived in UK as Tier 1 Dependent and her leave to remain expired back in mid 2012. Since then living in India.
Dependent 2 (Child) - Never entered UK since born.

Has anyone applied for the Dependent + Child visa from India recently as a dependent of a British Settled Person?

My Questions
1. Separate online Application form (Visa4UK) is required for child along wife?
2. Does VAF4A - Appendix2 form (for Financial Requirement) is required in addition to online form ? If required is it a separate forms to child and wife.
3. Does the sponsorship form (SU07) should be separate for child and wife?

Hi,

Can any one help me to answer what questions in section 1 and section 2 of Appendix2 form should be filled for a child of 8 years old.
I know section 3 is Mandatory since it is financial..
Confused about questions given in Section 1 and 2 which are related to spouse/partner mostly..
Kindly answer ASAP.
