FLR FP based on Child living 7 years in UK - Urgent Help Pls

Dear forum
The situation is that both parents (over 18 yrs) are applying FLR FP based on a child who was born in the UK and lived her for 7 years with extremely serious medical condition and is disabled. They are paying 1 main applicant plus 2 dependants - how many forms do they have to complete?

Would it be 3 separate forms? Does each applicant have to fill a separate form like they each have to pay a separate fee and NHS charge.

is it required that the 7 year old boy also submit an application or is it enough that he is a dependent on his parent's application forms?
Casa? CR001? Obie?

It will be just one application form. One of the parents will be the main applicant. The other parent and the child will both be dependants, but the application will be mainly focused around the child and why he shouldn't leave the uk.
