Getting back after 3 years seperation with my wife

Hey
I am planning to apply for a UK spouse visa from india in Feb 2017.
I got married in uk on sep 2012 and we stayed together till jan 2013 and then we got seperated. I didn't wanted to overstay so i came back to india on feb 2013.(Note : We have never been legally separated or divorced)
Now we are in relationship again from april 2016 and my wife came to meet me twice this year as well, also we have whatsapp messages and pictures to prove our relationship.

What else documents do we need and would HMO question about us getting seperated?

Any suggestions will be very helpful.Thanks
