Documentation and form to apply for my son to join me in UK

I will like to know the correct form and documentation requirement to bring my son to join me in the UK.
Summary

Son born abroad
Father UK Citizen
Mother absent
Being taken care of by fathers mum (grandma)
Father has been supportive financially
Grandma is now very old Over 70 year old to look after him going forward.

What form and documentation is need to make an application?

Where is the mother?

Do you have official evidence of some reaponsibility?

How old is child?
