TIER 2 extension Paper form / online

Dear Friends
I wish to apply for my tier 2 extension .
Couldn't download PDF form on the UKBA website. Also two online services found 1) BETA trial 2) apply.ukba.homeoffice.gov.uk.
Can someone please guide how to apply.
My Visa is going to expire on 30/04/2015. And I Wish to use Premium service.
Can I apply on 27th April 2015 ? Or is it too late ?
1) is paper form application still accepted ? 2) can anyone provide the correct link to apply online ?

I am not sure why are you not able to download. You can download from below link.

Yes, You can apply before your visa expire.
