Spouse Visa Extension 10 year route

Hi,

my cousin's wife came to UK about 2 years and few months ago on a spouse visa. Her visa will be expired in May 2016. At the time my cousin was earning £20,000 a year but due to medical reasons he stopped work (He had a heart attack last year and also he is a diabetic). His wife work part time and she earns less than £10,000 a year. They are living in a rented house. My cousin never claimed benefits nor he informed the Department for Work and Pensions (DWP) about his unemployment. My cousin is a British National.

As they both earn less than £18,600 they cannot apply for a 5 year route. Can anyone please tell me if they can apply for a 10 year extension visa?
if so is there any financial requirement?
does she require English Language course if so what level?
and if they cannot meet financial requirement is there any other way?

She can submit an FLR(FP) application on the 10 year partner route to settlement. It's essential that she applies before her current visa expires. My understanding is that A1 English isn't required under the partner route, but wait for confirmation of this. She will have to pay the £500 NHS surcharge in addition to the visa fee.
(Casa, not CR001)

What would be the consequences of an FP application a few months after the initial spouse visa has expired?
