Tier 2 General - Name & Passport change after marriage

Dear experts,

I'm in the UK and I have a valid Tier 2 General visa until January 2020. I got married and my surname changed. I renewed my passport accordingly with the new surname but the valid Tier 2 visa is on my previous passport and on my previous surname.

I have two questions:

1)It was impossible to get advice from the immigration office via phone but I've found the "Transfer of Conditions" (TOC) from on the web. First of all, can you please confirm that this is the right next step?

2)There are two options for the application 1)via post or 2) via premium service centers. In order to fasten the process I wanted to get an appointment for the premium service but the fees doesn't seem right for my condition. Normally on the TOC Documents the fee for an applicant is £223 and the appointment booking fee (premium service centre) is £100. The total should be £323. However when I try to make an appointment online the fee is £723 which does not seem to be right.

Have you got an experience/knowledge on this? Can you please comment?

It is £500 to apply in person at the Premium Service Centre, not £100.
