HSMP Tier1 extension, Salary points

I have HSMP 1+3 years and looking for extension. I need 5 points more to score 75 points under HSMP tier1. My questions is;
Can i show two jobs (one permanenet and other part time) and score salary points? if yes please describe how?
How many months bank statement should show for HSMP extension?
if i get another job of £1000 per annum then i can score 5 more points and can qualify 75 points.

Yes you can show more than one job for salary.
It is clearly mention in the guidelines.

For documents you might need Payslips, Tax Returns, Letters from all employers stating your employment status.

For the duration it would be any 12 months period in past 15 months.
So you can choose any 12 months (Choose the period in which you earned maximum) out of last 15 months.

I have almost 4 months in my extension and if i start a second job then i can only provide second job prood/ details for 4 months not for 12 months.
As you mentioned:

So in 12 month time if i have one permanent job and second started in last 4 months.
Is that will accept Home Office?
Second job need to be in profession?
