I am in the same situation as Sneha.
Wife got ILR through Tier2 5year route and I am on Tier2 dependent since Nov2013.
I need one more year to be eligible for ILR.
So which form should I fill, please? Also, i have found in the point 3 in your reply that form is for Tier 1 or 5. So can i still use that form or else any other form?
Please guide.

5. Yes, as long as your visa is valid when you return. There is no requirement to have a visa valid for a certain period.
6. Yes, as if you apply by post, you have to send your passport. You can also apply in person for a same day decision but this costs a bit more.
