Help on SET M form!!!

Hi,

My husband is due to apply for his settlement under the 5 year route. My husband’s annual income meets the income threshold.

1. Do I need to also add my employment? I am a British Citizen here in the UK and employed full time before I went on maternity leave.
2. I am currently on maternity leave until July 2019, do I need to get a letter from my employer to say that I am on maternity leave?
3. Would it be a problem if I didn’t include my income on there?
