Financial requirements for FLM

Hi, I am applying for an extension of FLP M

I am the director of my limited company. In my last Tax return all profits stayed in the business account. I didn’t not withdraw wages. My wife was employed at this time. Since this return I have been paying myself every month. Next tax return is due next month and visa this month. How do I show I am the director on this form as I only get an employed option and what documents will I need? My wife also receives PIP.
