Question about dependant application

HI all, firstly I would like to thank you for your brilliant effort.
I have a question- I am applying for HSMP to Tier 1. I have my wife and a 3 year old child as dependants, and am in UK for the last 2 years. Do I have to fillup a separate form for dependants? I dont remember filling up a form for my dependants last time- just gave the names as far as I remeber.
If I need formsfor dependants- is it 1 or 2 forms? And can I send them all together with my application?
I believe the total fee will be £920.

yes, separate forms for each individual - to be sent together - all photos to be placed in one envelope in the main applic. set of docs. fee is £920.

One is actually expected to check forms a day before posting just to make sure they haven't changed overnight!
