Tier 1 general extension on salaried based employee basis

Hi,

I suppose to apply for Tier 1 general extension on salaried based employee basis, during March 2010, please advice the following.

1. As I understand I need to attach following documents along with my application I will appreciate your input based on your experience.
a. Covering letter
b. Application forms main form for myself and dependent form for each dependent
c. my education degree,
d. HSMP original approval letter from home office,
e. Self Birth certificate, (I am not claiming age points)
f. marriage certificate,
g. children birth certificates,
h. my pay slips for last 12 months,
i. My bank statement supporting the payslips and confirming maintenance fund.
j. employer appointment letter

Please advice on which address am I suppose to courier my application along with the forms.

c. my education degree, Not necessary if you have already proved with initial HSMP application but, to be on the safer side you can send it
d. HSMP original approval letter from home office, Not necessary
e. Self Birth certificate, (I am not claiming age points) Not necessary
f. marriage certificate,
g. children birth certificates,
h. my pay slips for last 12 months,
i. My bank statement supporting the payslips and confirming maintenance fund.
j. employer appointment letter Not necessary

Please advice on which address am I suppose to courier my application along with the forms. address is on the first page of the form. Postal address and courier address are different so make sure you send it to the correct address

I will appreciate your assistance.

You should also send 2 recent photographs for each applicant. They should be taken within the last ,month from the date of your application.

Thanks for the reply, however I will appreciate detailed response to makesure there shouldn’t be any issues with the application.

As I understand I need to do submit following for my HSMP to Tier 1 extension, I like to make sure that my application is fully complete to cover the case of my self and three dependents (wife + 2xkids).
a. Covering letter
b. Completely filled and signed FORM “tier1generalguidance “ for main applicant
c. Completely filled and signed FORM “pbsdependantapplicationâ€
