Got my Tier 1 visa from HSMP........

Here is the time frame

Application Submitted : 21st Jan 2010
Application received @ HO : 22nd Jan 2010
Payment Debited : 27th Jan 2010
Reference number received : 30th Jan 2010
Visa Issued : 17th Feb 2010
Doc received : 20th Feb 2010

Doc submitted
Covering letter
12 bank statements
12 Payslips
Letter from Employer

what form did you fill to HSMP Extention?
Is it same for as Tier 1 ?
