Tier 1 (G) - Application forms - Please Advice

Hi

I am applying for Tier 1 (G) visa from India.

Can someone please confirm that the below application forms are correct / do I need to use different forms?


1. Application form VAF9 - PBS Migrant
2. PBS Appendix 1 General Migrant self assessment form
3. Print out from PBS Self assessment calculator


Plus I will be enclosing the below documents as well

Covering letter
2 photos
Fees
Passport
Educational certificates
Bank statements
Pay slips
Letter from Employer
Letter from Bank


I am bit concerned on the application forms.

After looking at the website it looks that i need to enclose the below forms only.
