Dependent Form and accommodation proof?

Hello,
Thanks once again for all your help.
I just need your support on two more things.

I’m applying for Tier -1 for myself and my wife(as dependent).
1. Can someone please assist letting me know where can I find Dependent Form?

2.I’m on TIER -2 and switching, Do I still need to provide Accommodation Proof?

If so, can Council Tax proof will work?( it’s on both’s name) OR do I need the tenancy agreement?

never heard of proof of address requirement, specially for an in-country application
