In US, immigrating to UK (Tier 1- General)

I am currently working in the US and want to immigrate to UK. It appears that people in US can submit online application and pay fees online.

QUESTION1: Once I submit the online application, pay fees and do my biomterics, how much grace period I have to submit supporting documents and where I should submit them?

QUESTION2: Also, does the visa application fees change on April, 2010. If yes, is it getting cheaper or more expensive?

Fees - when you apply online for appointment
Biometrics, documents all - when your appointment is
Location - while applying online you will have asked to choose where you want to apply from.
