I have been self employed since 2005. I am an EEA national too and my application for naturalisation as BC has just been granted. It only took about 2 months. I also have the Permanent Residence Card (EEA3) issued in March 2011.
What I sent was:
-a cover letter explaining my circumstances just to explain that as self employed I did not need to register under the WRS which only applied to employees.
- My permanent Residence Card (blue little book) just to show them that I am a Permanent Resident in Uk even though this was not a requirement.
_ Inland Revenue self assessment account for the last 5 years showing that I have paid taxes

My EU passport but you can also send your EU country ID

completed application form
Bear in mind that you have to hold Permanent Residence for at least one year after being issued the Permanent Residence Card.
Good luck

Also I forgot to mention not to waist your money or time using the NCS as they do not have a clue. They gave me the wrong information and demanded I sent this and that for my application. So I decided to do it by myself writing a cover letter explaining everything and it has been successful. I'd recommend you do not leave the country in case they need to contact you, but you can send your ID. Which country are you from?

Have you answered questions 2.4-2.6 or have you only included PR confirmation?

Hello. Here is my experience but I applied through NCS, I have a limited company, which means I am self-employed, but I also pay myself salary as employee and I am a director of the company. I do not know if as self employed you pay your self salary or not ( I am guessing not).
I took with me the P60, P35 and self assessment documents for paying taxes. I also ticked self employed, director and employee as the three of them apply to me. I added my employee number and UTR which is the tax number for company on the application. When I went to the NCS, The lady said you do not need any of these documents as the prove of income is an alternative prove in case you do not have passport or PR or more than one year, so she did not take any of these documents and said it is enough you have your payee number there.

The other documents I needed were
1- Passports ( two of them as one expired during the past 6 years)
2- UK life test
3- PR document ( more than 1 year since date of issue)
4- Driving license which function as prove of address. I had another prove of address like electricity bill and bank statement, however the lady again said it is not needed and your DL should be more than enough
5- Receipt of payment for the 874 GBP including ceremony fee ( print it from the UKBA website)

That is all I needed

P.S I also did not complete section 2.4 to 2.6 which is linked to EU members as I had my PR for more than one year

Hope this helps

Just the problem is with my Revenue .
I call on 10th September to Revenue for Self Assessment Statement for a last 5 years and as well I send letter to National Insurance for a Statement that I was pay my Insurance since 2007 but still waiting for a letter.
I called to Revenue and they say that send me the statement on 11th September, and they will send again.
Today I find form to send for NIC on they website. And they say will take several weeks....I do not have time if I need send all off them until 25th October...
So my question is.
Can I send my Self Assessment Statement I've downloaded from them by the HM Revenue website???Will be valid???And send my Self -EMPLOYED Class 2 NIC bills ???
wHAT i CAN DO???
Thank you so much.

I am not sure if you need 5 years worth for prove of self assessment. I believe they ask for only your last year ( but I could be wrong). For your self assessment, I can not give you a confirmed opinion ( but do not take my word for it as I am not the expert here). I think it may be enough, but if you want to be 100% sure, you may want to ask UKBA or HM Revenue
Sorry could not be of more help
