as its been a year she joined me so i was confused as obviously in her last application HO would have assigned her a reference number !! so if yes where can i find it or owise shall i mark NO ?

!!! 1 Important question , my accountant flew away so i changed to another accountant and RTI has not been done. is it okk if i dont have RTI submissions for this ? and IN form Section N it says WHAT DATE DID YOU BEGIN SUBMITTING REAL TIME INFORMATION TO HMRC ? what shall i fill in this date column ??
- RTI is mandatory. Without that you can be ensured of a rejection

If you are self-employed when you apply, you must provide documentation to demonstrate that you are registered as self-employed with HM Revenue & Customs.
Your latest tax return should satisfy that if it was issued within the last three months and it states you are self-employed.
As mentioned, investment evidence is missing.
FPS is absolutely needed - other users seem to have used a software called Moneysoft to recreate the FPS.Talk to your new accountant.

These are much easier to define in case of a limited company, but the requirements are still the same for self-employed. More details on each of these are in the guidance, but it is really the first one that is missing from your document list. The others should be ok.
You need something from HMRC within the last three months that shows you are self-employed still. Filing the latest return is one way of getting that, but if you have got a letter from HMRC asking you to submit your returns, that would work too.

I am not absolutely sure about this, but I think Tax returns are not sufficient. And anyway you need a compilation report from your accountant. Not sure if the accountant provides that for your tax return.

Ask your accountant - they should be able to generate an accounts for your self-employed business
