Entrepreneur Tier 1 extension to ILR need Help

The other thing which i needs to clarify about consent form. I've left blank page no 2 and 3 as they don't applicable on me. Should i leave it blank or do I just write N/A on it?

I'm applying for my wife's further visa. Her current leave is expiring on 02-05-2019.

She will qualify for ILR in September 2019. Now I have to apply for her PBS dependent visa in order to complete her 5 years.

How much IHS I have to pay for her? isnt it £1200?

If I choose Route Tier 1 and Visa type Tier 1 Entrepreneur(extension 2 years) and go ahead than Payable IHS is £200.

Please keep in mind that I've applied for ILR but my current leave is also expiring on 02-05-2019
