Urgent - Formats & advice required for Self Employed

I am on HSMP visa and going to apply for extension to Tier 1. I need help in the following:

1- If anybody can please send me the formats of Sales Invoice Summary and Accountant's letter etc.

2- I will be sending Profit and Loss A/c, Balance Sheet, Accountant's letter, Sales Invoices Summaries, copy of Sales Invoices and Bank Statement confirming receipts against invoices issued. I will not be able to send any Income Tax related documents as the tax year is not completed and hence no records available. Please advise will this be ok for claiming points in prev. earnings.
