HSMP Extension Form HELP

Hi,
I need help on following in HSMP Extension Form :

1) PAGE 11
B10 : Home office reference number: ??? Is this is the same as i have on my aproval letter ??

but they demand two

Number 1: ???
Number 2: ???

2) PAGE 11
B11: any previous worker reference number ???


3) PAGE 11
B13: Point Base System Migrant reference number ???


4) PAGE 20
D4: Please enter the reference number on the entry clearance or visa which u can found in the applicant passport ????


5) PAGE 21
D11 : What is applicant currrent Immgration status in UK
Is this is HSMP ???

6) PAGE 55
V3: AWARD Reference number:

This box has total 13 segments.........and IELTS report has 18 digits number ........how can i write it ???????

--> Had you ever been on WP? if yes fil the Worker Reference no in the WP letter or Leave it Blank,

3) PAGE 11

--> Leave it Blank

4) PAGE 20
D4: Please enter the reference number on the entry clearance or visa which u can found in the applicant passport ????

--> Red no on the top right on the EC Stamp

5) PAGE 21
D11 : What is applicant currrent Immgration status in UK
Is this is HSMP ???

--> Yes

6) PAGE 55
