EC Questions

Hi,
I want to apply for EC from Pakistan. Plz help me by prividing answers of the following questions:
- In VAF1 Form, no option for HSMP visa, should I check "Work Permit Holder" box ? Any time duration for visa should be mentioned ?
- If I apply with my wife and children, my dependents need to travel to UK within 3 months ? (as normally 3 months limit is defined) or 3 month time limit will be applicable to principal applicant ?
- If I apply with my wife and 2 children, I have to submit 4 forms, should I fill every section in 4 forms or 3 family sections and one with out family-section ?
