renewal of BRP after surname and passport changed

Hi,

My wife got my surname after we got married (2 months ago) and finally we are managed to change her passport, this new passport has her new surname now.
BRP still has the maiden surname, we need to inform UKBA (home office) for this and probably renew BRP for that matter.
Can you tell me What form do we have to fill for renewing BRP ?
