Limited Company - Income Calculations

Hello Members,
I am self employed, director of a limited Company. I get financial statements from my accountants on A4 paper which shows dividends . My question is, can i use these statements as evidence for my income along with the accountant letter ? Or do they need to be on any letter head ?

Does the company have a Company Secretary? If so who is that? The Accountant?

What you really need are dividend vouchers, showing the amount of the dividend and the tax credit.
