TierOne Extension query

Hi,

i am going to apply Tier1 extension in Mar 2014. I am self employed & I am not sure if i can show only dividends (not salary) for earning points??? Do i need to take out letter from bank for mentioning the current balance in my bank account or can i just statement will work??
If anybody knows the answers please let me know asap ..

If you are an IT Contractor (Ltd Co) you are not self employed and as such will need to provide your salary slips and dividend vouchers.
