Separate application forms for ILR

Hi,

I am due to apply for ILR in May' 14, based on 5 years stay on Tier 1 (General).
My wife joined me in 2009 so she qualifies for ILR as well and will apply with me.
Please confirm if I can just use ONE SET (O) form for both of us or do I need to fill in two separate SET (O) forms, one for myself and second for dependent ?

Help much appreciated

If she is on Tier 1 Dependent visa, this is fine.
