ILR application Form for PBS Dependent

I came to UK as Tier2 Migrant and then my spouse came as my dependent(5 years back). I got my ILR via Premium service last week. My spouse depedent visa is valid till Dec 2014. If she has to apply for ILR now( she had completed her qualifying period) which FORM should she use? SET O or SET M. By the looks of it it seems like it should be a SET O form. I would like to get this confirmed.

SET(O) B1 English and LIUK required.

What about cohab documents for set o form applicant pbs dependents.? Or only English B1 and lifuk would be sufficient.

Cohabitation should be evidenced since the last extension up to a max. 2 years.
