Please see my timelines below and advised if I am able to apply on 6th June 2014 (next month):

* Out-of country visa approved (based on out-of-country work permit): 02/07/2008
* Entered UK: 08/08/2008 (around 1.5 month delay from EC)
* In-country visa based on Tier-1 (G): 01/07/2009
* In-country visa extension based on Tier-1(G): 01/2012
*ILR on (work permit +Tier 1(G) stay): 05/06/2013

I was in the UK in June 2009 which will be 5 years from date of application.

Q2: Please right form or link to form (if possible at all!)

You can apply on or after 6th June 2014. provided you fulfill all other requirements.

If you have been resident in the UK for the past 5 years, have ILR for one year and meet residence and good character requirements than you can apply.
