ILR Tier 1 need urgent help checking elligible

Hi,
I entered UK in 2004 on student visa then switched to Tier 1 post study work, then Tier 1 General visa.
I had initial Tier 1 General applied within UK date of issue 13/Feb/2009 until 13/Feb/2012 then I applied for Tier 1 General extension before 13/2/2012 but unfortunately it was rejected, but finally got extension through in reapplications during appeal processes on 05 April 2013 (date of issue printed on BRP). So I have Tier 1 General extended until 05 April 2015.

Can I apply for Indefinite leave to remain (ILR) on basis of Tier 1 General in the September 2014?
Does period of 13/2/2012 to 05/04/2013 towards 5 years residency under Tier 1 General?
Can I go for premium service for same day decision?
