help with an form please

Good evening all.
I know this question had already been posted from my.ex wife.but she forgot to mention some details. In the form it does say marital status. Legally I'm still.married but I'm spirited from my ex wife now I got a.girlfriend and we waiting to have a baby. So the question is in the section where it does say details of partner shall I put.my.current girlfriend and my ex wife in the section for previously married?
Also date of when ilr has been issued. I been issued the visa somewhere in July two years ago however in September I did apply for.isd card because they had printed my visa on.paper as my.passport had expired at that time so shall I put when my first visa has been issued or when my isd card has been issued.
Also forgot to.mention that I tried to get life.in the UK test certificate copy from my solicitor or even the number of the test no success would this be an issue. Thanks and appreciate any help.
