help regarding FLR(M) form

our situation is
My husband is a UK citizen and i am under a work permit. we married in July 2013 back home. we are planning to book an appointment for the FLR(M) Application. i have a couple of queries regarding this..

SECTION 6 YOUR RELATIONSHIP TO YOU SPONSOR
we have been living together for 5 months now.
question 6.8 asks how long have we lived together and after asks to add all addresses u and ur sponsor have lived at in the 2 years preceding this application if u have lived at ur current address with ur sponsor for less than 2 years.
Should we add the address where we lived separately before our marriage in uk ?

SECTION 8 ENGLISH LANGUAGE REQUIREMENT
I have done my IELTS (ACADEMIC) in 2011 and now it has expired. is it still accepted as evidence that i have met the English language requirement or should i repeat the English language test??? what would u suggest me to do?

What category of work permit have you got and how long have you been in the UK on it? When are you eligible to apply for ILR under your work permit?

Was your English Language test result used in a previous successful application?

I am under tier 2 general visa and working as a Registered nurse from 2012 on wards (2 years completed).

One of the requirement for my tier 2 visa was IELTS and i passed my IELTS from Aston University, Birmingham in 2011.

If you have already spent two years under Tier 2 (General), then you may require just another three years under Tier 2 (General) before being eligible for SET(O). However, if you switch to FLR(M), then you will require at least another 5 years, from the grant of FLR(M), before being eligible for SET(M).

Therefore, if you are able to apply for SET(O) under Tier 2 (General) in three years' time, then it may be better to remain under Tier 2 (General).

i am not eligible for PR ...so im going for dependent visa as i got my work permit in 2012.
I have a doubt in section 6.9
If relevant have u lived together permanently in the uk with ur sponsor since ur last grant of limited leave to remain as a partner??
With an option yes and no
well im living with my husband frm june 2014 and this is my first time application for dependent. Kindly provide me with the reply

I think that question 6.9 is not relevant to you, as you have never been granted leave as a partner before.

Why would you not qualify for ILR under Tier 2 (General) in 3 years' time?
