I wrote this also when I was applying for a visa, based on my Green Card.
I addressed it with "To Whom It May Concern", I see that's the usual form of address in this part of the world.
I wrote something like:
"I have been granted a Green Card, number XXXXX, valid for the period XXXX to XXXX, in order to start employment with company XXXX. I am thus applying for a long-term employment visa, in order to be able to land in the State.
I have an aunt in country X, her name is bla blah, her address is XXXX. I also have a cousin in country Y, his name is bla bla blah. (Didn't have his address.)
I will be staying at a B&B (I have not yet made reservations, until this visa is approved), while looking for permanent accomodation.
I state that I will observe the conditions of my visa, and do not intend to become a burden to the State. I will leave the state on the expiry of my permission to remain."
If your employer has already provided the accommodation, you should definitely give the address. Or, if they will be paying for a hotel or something, it would be good to have a letter from them confirming they're going to do that.
