Reapply/Extension of family visit visa

Hi,

I am on Tier1 General visa and my In laws are in UK with a 6 months Visit Visa. My wife require some urgent medical treatment for some time and Is there a way to extend my in laws visa after 6 months from UK itself.

If not, can they re apply for family visit visa for another 6 months immediately after going back to India (is there a restriction that they should be in India for some time before applying again)

Please let me know, Thanks a lot
