About EC -VAF1

I have got HSMP approval and now I have to apply EC for myself , my wife and two daughters aged 5y and 1y respectively.

Do I have to fill the VAF1 for each family memeber seperately or I can mention them in my application form?

Secondly , do I have to pay fee seperately for each family member and is it 200 pounds even for a 1y old?

Here is reply from High commission about me and my wife and my 6 month old daughter.
