I don't understand which documents I need to provide

My case seems to be a no-brainer yet it's very confusing. I came to the UK in 2005 from a A8 EEA group of countries. I registered with WRS in 2005 and worked continuously with the same employer for 8 years. I have p60s to prove it. I also have all the diplomas they ask for in regards to the knowledge of the country and language. My only fine is now spent. Over a year ago I was let go and decided to take year break. During that time I stayed in the country with only short periods of absence. Now I'm back looking for work and I'm registered with JobSeekers. I don't understand what documents to submit. For example.

My passport is new and only covers last 3 years, do I need to prove the last 5 years despite PR status?

Do I need to fill in section 2.4-2.6 of the form, isn't PR status supposed to be automatic? Also if I do need to fill in this part, they ask for the last 6 years whereas my continuous period was from 2005-2013, what should I put that I was doing in the last year?
