Financial requirement: Child dependent visa

Hello Friends and moderators,

I have become a self employed contractor from July 2014 and paid to myself when ever i need. Is this alright or I should be showing fixed salary drawn every month?

Salary + Dividend i took from April 2014 to March 2015 : £40000+
Previous income 2013-2014: £40000+ (Permanent employee)

My child is born 4 months ago in India and i am applying for dependent visa.
Me: British (Naturalised 2 months ago)
Wife: ILR

My question is, Is it ok to show the salary+ Dividend which is drawn on different intervals as my income? If yes then how should i generate the pay slips?

Do i need to fill the VAF4A2(Financial Requirement) form? I am a bit confused whether i need to fill or not.
