Settlement form to fill- SET (O) or SET (M)

Hi,
I am a little confused with the name of the application form I need to fill in order to get ILR for my dependent wife.
We applied for the ILR last week together using the Set (O) form.
But her ILR got refused because the english exam she cleared was not the one which is acceptable.
So, I got the ILR but she didn't.
I need to apply for her ILR again.

Can someone please help and let me know which form I need to use for her ILR application now?

I am still confused though. As I have got the ILR now, what should I fill in the main applicant part in the Set(O) form?
I just want to apply for my wife's ILR.

Fill your wifes detail as main applicant.
