Dividend Statement Template

I am applying for the HSMP and need a dividend statement template to prove my income from my limited company earnings

Does anyone have one?

Why don't you ask the Company Secretary of your company to produce the needed dividend voucher(s)?

regcoats, if your company secretary or your accountants can help you with the format, they would be the best people to do this for you. Most accountants will help arrange this.

But if you are in the same situation as I was - with your accountants are being exceptionally unhelpful and your company secretary needing some advice - send me a PM (private message) with your email address and I'll send you the template I used. It worked fine for my HSMP extension in Jan this year. Not sure if there have been any changes in the requirements for dividend vouchers since then, but I don't expect so.
