Need to extend two year spousal visa

Usually I know exactly what's going on with visa's but even I'm lost.
Because I am on the 5 year path I am coming to the end of my first two year spousal visa which was issued to me.

From what I have researched, I need to now apply for an extension but when searching for the relevant paperwork, the paperwork that seems to be coming up is for people who want to apply for ILR. I just need to extend it.

Please can someone post a link with to the relevant paperwork.

Extend for another 2.5 year period by applying for FLR(M).

You will also be required to pay the new NHS surcharge of £500 online before submitting your application.
That's in addition to the visa fee of £649 for a postal application or £1049 if applied for in person.
(Casa, not CR001)

Hopefully this is just a typo and you actually mean 2.5 year visa or 30/33 month visa.
