I am all set for my EC. But I see that a new form that has come up. Could you all please share your experiences on the new form. Also it puts more stress on the job offer. Will that affect us?

Ok let me start this by putting my doubts:
Disclaimer: questions may be silly please bear with it

2.3 issuing Authority: Republic of India?

3.1 your address: can this address be different from my HSMP address(I used my friends address for the HSMP) <Is it a safer bet to use the same address that is my friends address?>

3.6 contact details: I am not filling it up (keeping it same as 3.1)

6.1.4: details of qualifications: certificates <what else can we put here?>

6.1.5: details of work experience: <just mention payslips/ITR?>

6.4.13(monthly income given to family) and 6.4.14(monthly expenditure on living cost) <what is the amount to be put I mean we can just aboout put anything rite?>
