As per guidelines urgent treatment form can be sent through fax once you have reference number and that is the reason that column of reference number is appearing on urgent treatment form.
But I have a precedent where urgent treatment form was sent along with the HSMP application form of an in country applicant and the same was entertained.
HSMP application was sent on 13 Nov. and approval letter received on 2o Nov. is dated 16 Nov.2007.
Method can be resorted in appropriate cases.
