HSMP form1 - Help

Hi, can anyone help me filling HSMP from1?

I have downloaded latest HSMP from1 from http://www.workingintheuk.gov.uk/workin ... .html#HSMP

In MS Word version, there are some fields which can’t be editable, can anybody please tell me how to edit them?

Is there any other source of latest editable version of HSMP Form1?
