naturalisation certificate missing name at birth

naturalisation certificate missing name at birth

Hi,

I am appear to be at the same situation with my Naturalization Certificate having "Name at Birth" blank. Although I was sending HO my Birth Certificate.

Filling up passport application I wonder in the Section 2 "Who is passport for?" if I leave "maiden or all previous names" blank as well to match my Certificate. Just fill up surname and name as it is on my foreign passport and Certificate.

Can I do that, or I have to write my name at the birth, even if I don't use it for over a decade. The problem, is that I got married back in home country, divorced, that marriage certificate has gone long time ago in a bucket( unrecoverable).

On my naturalisation certificate the 'name at birth' is also blank. I had no problems with my passport application including my maiden names in section 2 etc. You need to complete the forum truthfully. HMPO can check if they want to. It is not an issue to include the maiden name even if it is not on your certificate.
