Dependant of Long Resident ILR

I need a help..

I am in Tier 2 dependant and my husband is submitting ILR application via premium in Feb...My questions are

1. Which form do i need to use?
2. Do i need to show English qualification ( I am an MBA from a reputed institute in India)? if yes, which exam should i write for my first extension?
