P60s, a couple of payslips from each year, a letter from your employer, bank statements showing regular salary payment are all good for proving employment.
You only need to list absences from the start of your qualifying period until present day, NOT everything since you first came to the UK. Don't worry if the dates are not 100% accurate, unless you have spent a considerable amount of time (e.g over 365 days) outside the country then it doesn't really matter. You could include a short covering letter stating that some dates might be approximate.
Processing times seem to be around 7-8 weeks at the moment. Good luck

List your employment to present day if you like, it makes no difference. But evidence is only required for a 5 year period, so no need to send most recent P60.
You don't actually need to use the EEA(PR) form to apply. It could be worth having a search around the internet and seeing if you can get the old EEA3 form; it is much shorter and simpler (the EEA(PR) form is the amalgamation of about four different forms into one). UKVI cannot refuse an application made on an old, or even no form (yes you can just write a letter).

Using the ID card for PR certificate but passport for citizenship application is not a problem. I believe HO expect you to use the same ID for Life in UK test and citizenship, but nothing mentioned for PR.
If you are travelling inside EEA, can't you use your ID card instead of passport anyway
